My feet have a tendency to spin out right at the start of the second pull -- the problem is more pronounced at heavier weights. I have seen this on other lifters before, but have never seen a diagnosis or solution. Any ideas?

I've also seen this in a some lifters. It's kind of hard to diagnose. I think sometimes the problem is from people shifting their weight too far back in the 1st Pull then making an overcompensatory adjustment to transition to the 2nd Pull. If that is the case for you then try to balance your weight more evenly across the foot (more towards the middle of the foot).

Another option is to try to start pulling with your feet at the position you shift them out to.

Try doing some flat-footed Snatches/Cleans (sometimes called "Snatch/Clean [no feet]"). Keep these light. You can use them as a warm-up It will feel weird but it will help.

Also make sure you're maintaining constant pressure against the floor throughout your pull - never let up on the push against the ground.
