Default ketosis

I have been eating paleo for a while with great results but sometimes when I go mostly veg with no fruit I get super thirsty and heard that may be a sign of ketosis? Is this bad should I always try to keep my carbs above a certain level? I think I heard ketosis is hard on the liver or kidneys is this true my main goal is health.
