The toes turned out was my problem for a while. It's hard to look down and see your toes when you have the weight racked across your shoulders in front. Also, when you do a heavy clean (full squat) or snatch you don't usually look down to see where your toes are. But chances are your toes are out more than you think.
I recently realized I had been flaring my knees out when I squatted much more than my toe flare. In other words, my toe flare didn't match my knee flare. On top of that my feet are already slightly pigeon toed to start. For example, if I am just laying down in bed and I straighten my legs, my toes are pointing out slightly.
