Baby steps, man. I worked with a very large guy (close to 500 pounds). He was a former athlete but completely detrained. After a diet overhaul, I started him on a simple plan: walk down the street every day, get up off the floor 5 times, and do 3x5 wall presses). 80 pounds later, he's worked up to stripper squats, barbell work, and short circuits of grappling.
I agree, though. It's a sad state of affairs when we need an on-ramp program for walking.
