Okay this may seem nit picky. Your set up seemed rushed. Get under the bar like you were going to squat right there. Take a deep breath and un rack it. Take one step back far enough to clear the rack. When you are coming out of the hole you pitch forward. When I was doing that I had to try hard to keep my elbows up and that seemed to keep me up right. Dan John mentions thinking about torso being slung between your legs rather bending at the hips.
