When you say to flare the knees out for the pull, how much external rotation of the feet should be utilized at the same time? How much can the knees go out of line with the feet?

The feet should be rotated to the normal degree - the knees will flare beyond that - they will not be aligned like they are in the squat. The knees can move significantly outside the feet if you're flexible enough (which you should be).
