I think you rushed getting into your second pull. You could benefit from sweeping the bar back a little more to avoid that hop forward. It's a timing issue. Segment pulls (1", above knee, and at the hips before you go vertical for your second pull) might help you with correcting your muscle memory. Break it down again. You're more than strong enough, so I don't agree with that comment of the weight being too heavy; you simply need to correct your technique. Therefore, use the segment pulls or go off blocks to help you understand what you should feel and when you should feel it by breaking down the portions of the lifts.
