Bathtub full of cold water, then add ice--I've been using 35-45#. Stay in there about 20 minutes (though I've heard as low as 12 is sufficient). Acclimation is easier if you get in the cold water and then add the ice on top of yourself.

About an hour afterwards I follow up with contrast showers or a warm shower as I heard that will help your muscles not cramp up from the bath.
