Default USA Weightlifting Certification Question

I'm hoping to attend the USA weightlifting Sports Performance Cert. in June in PA and had a quick question pertaining to the application. After the personal info section it asks for which USAW Club name you wish to be affiliated with...is this essential, and if so do I have to say that Rob is my boss and I work for him or do I have to register my gym as a club and pay resulting fees? I know it's a relatively random question, but I figured this was a decent enough place to ask seeing as how you're all O-lifting nuts anyway
